How to use hreflang properly
Hreflang works as a reciprocal set: every URL you listed above, including the page you are on now, must appear in the tag list, and the same exact tag list must go on every one of those pages — cannot just put on one page only.
Add one x-default entry to point at the fallback page shown to visitors whose language dun match any of your other versions. Strongly recommended, but not a hard rule.
You can put hreflang as HTML link tags in the page head, as HTTP headers, or as xhtml:link entries in your XML sitemap. Just pick one method for the whole site and stick to it, don't mix and match across pages.
